Applications for List Closure
The LMC is aware that due to workload pressures and in the interests of maintaining provision of a safe and quality service to existing patients, practices across the County have or are giving increasing consideration to closing their lists to new patients.
It seemed timely therefore to remind practices about the regulations around applying to close lists, which require approval from NHS England. Closing your list without this approval could lead to a breach in contract and the document Closed List Applications GMS Model Contract 2014 details the regulations in full. They also set out the responsibilities of NHS England upon receipt of an application to close a list.
NHS England will not consider any application without receipt of the information required as per the Regulations, the most important being evidence that you have:
a) Discussion/consultation with your patients; and
b) Discussion/consultation with your neighbouring practices.
Furthermore, for your information (with the permission of the practice concerned), is an anonymised example of a list closure application, which was successful. Whilst the reasons for the application are pertinent to that particular practice, the format may of the application may prove helpful. Please note that contract references within are for the 2012 GMS Model GMS Contract and whilst the provisions haven’t changed, those available from this website are for 2014.
