The new QOF business rules/read codes

It explains why the changes have been made.
Appendix 2 highlights all the changes to existing indicators. This is not published anywhere else and should make it quite clear where existing indicators have changed and why the numbering is no longer sequential. This should help focus on the work practices will want to do in preparation for April 2006.
